But tear trough treatment is not straightforward, and approaching it as though it were is one of the most common causes of unsatisfactory outcomes in cosmetic injectable practice.

What Is the Tear Trough?

The tear trough is the groove that runs from the inner corner of the eye downward and outward along the border between the lower eyelid and the upper cheek. In younger faces, or in faces with good mid face support, this transition is smooth and gradual. As volume diminishes in the mid face, as the ligamentous structures that define the eyelid to cheek junction change, or in individuals who have a naturally prominent tear trough regardless of age, the groove deepens and creates a shadow that reads as hollowness, darkness or fatigue.

The skin in this area is among the thinnest on the face and the underlying anatomy includes important vascular and lymphatic structures. This is why tear trough treatment requires careful product selection, precise placement and thorough pre treatment assessment.

Not Everyone Is a Suitable Candidate

Tear trough filler produces good outcomes in clients where the primary driver is volume deficit, skin quality is reasonable in the under eye area, there is no significant lower eyelid laxity and realistic expectations are in place. It is less appropriate where eyelid skin is very thin, where prominent orbital fat pads are pushing forward beneath the lid, or where the hollowing is primarily structural rather than volumetric. In these cases, filler can worsen the appearance rather than improve it. Your practitioner will assess which category applies to your anatomy during your consultation and will not proceed where treatment is not appropriate.

The Role of Mid Face Treatment

A significant proportion of clients who come in concerned about tear trough hollowing are best served by addressing the mid face and cheeks first. When the mid face is well supported, the transition from the lower eyelid to the cheek improves naturally, often without any direct treatment of the tear trough itself. Treating the tear trough directly in a face where mid face descent is the primary driver frequently produces an underwhelming result because the structural cause has not been addressed.
